2/25 Updates

Hi folks!

Just a few short weeks and we are on the plane to Italy!  Getting exciting!

Please read everything below!

Your travel company is sending you the latest itinerary in the next day or so, and it includes your hotel information, what meals are covered, what exciting places we perform, and what attractions we visit.

TRAVEL TIPS  This doc includes some important information and helpful hints.  Please read it through carefully even if you are a travel veteran.

FAMILY ASSISTANCE Please help us out with some easy tasks.  The spreadsheet contains some jobs for those staying here and some for those joining us in Italy.  Please insert your name where you feel you can help out.

Student Crews: We will have some tasks for the trip and will need student help.  Assignments are coming out soon.

Some of you have not yet done our FORM COLLECTION.  We need those ASAP--the MHS Health Office has to have them right away.

• We need to start collecting forms!

  • We’ll also need to get some signatures on important MCPASD-required forms.  These are required of every performer.

    • FORM 1: Conduct expectations

    • FORM 2: Permission & Medication Admin Info

    • FORM 3: Medication form [only do this one if you included meds on Form 2–note that this form requires a medical practitioner’s signature]

• TURN IN FORMS/PASSPORTS HERE

  • Print FORM 1, FORM 2, and FORM 3 from above [performers only]

  • Fill them out completely

  • Copy your passport [all travelers]

  • Scan them all individually

  • Attach them to the Google Form

  • The form is set up to be able to submit multiple times if you don’t have all the info at once.
